CENTRAL SYSTEM in SAP

If Central Instance and Database Instance are install together on one system it is referred as Central System.

                   If the install separately on deferent boxes it is referred as distributed system installation. In larger companies distributed installation is chosen. While installing central instance and Database Instance, most of the companies configure high availability.
